Got the same transitStatemachinefromOffline error like everybody else in here with v.10.6.5 and reveted back to 10.6.4 for now, which is still kinda running ok for me with the workaround posted above.
Reverting the hr.exe back to the old version wasn't enough though, had to get the boot.dll from a 10.6.4 installation aswell. So maybe the issue might be found in the dll-file?
But I haven't tested running v.10.6.5's hr.exe with 10.6.4 boot.dll.
